In NY State "Subscribing Witness" must be PERSONALLY known by the notary. Thus, useless virtually all the time.

Was going to suggest witnesses but I see that's not an option for NY (which I can only imagine must be a pain...).

Umm, huh. I can't think of a notary solution honestly, but I also have no idea what New York's notary laws are.

Since the lady was in the hospital someone there must be able to verify her identity. If so, perhaps they would sign as witness to that? I am not sure what form could be used but it should take care of the problem. Perhaps her doctor could help?

I get a call from the nice sounding lady who wishes to help a relative in a nursing home. The patient wanted some articles from their home brought to the hospital. Unfortunately, somehow the ambulance crew locked the keys to the patient's residence inside the apartment. The landlord wants a notarized letter of permission to allow the "nice sounding lady" access using the passkey. ALL of the patients' ID's are in the apartment.
