Where was the doc originated at? More than likely by an out of state title company. If it's being recorded in CA, then attach the Jurat with the correct verbiage.

Doesn't CA demand certain wording on the notary certificate? I'm not from CA but I think that's what I have read from other CA notaries. You have enough information from the preprinted wording to know which notary certificate is being requested (ack or jurat), however if the preprinted wording does not comply with any required CA ack or jurat language, then you should attach a compliant certificate, or for future use get ack and jurat stamps that have the CA compliant wording on them.

Or, if you can simply write in the missing verbiage on the pre-printed certificate, then that would work too.

Hello, I had my first loan signing and I was confused on a couple documents plus I was really nervous.

There was a document that had the notarial wording....

sworn before me this ____ day of _____, 20__.
_____________________
Notary Public
My Commission Expires

Do I need to attach a jurat? It seems incomplete since it didn't include 'personally appeared before me'
