In a word - yes.

You obtain two forms of ID, you complete this form, sign as Signing Agent - it's not a notarial act.

You have the blank copy because your notary only filled out the one that was returned - however, your notary SHOULD have asked your for a secondary form of ID, acceptable ones being itemized on the form itself.

Me again. I'm looking at my loan papers again and I'm looking at the Patriot Act which is requesting 2 forms of ID and once again mine is blank,of course I don't remember if the one the signing agent kept was filled out but the only ID I showed was my driver license. I have a couple of questions here. First, this is a form I would sign as the signing agent, correct? And if I sign it as the signing agent wouldn't I need to fill in the ID information on the one that that I give back to the lender since it says I certify I have personally viewed the ID? I am assuming since my copy is blank and printed from the same file and he did not ask for another ID so was the other one. Thanks again.
