I give one affirmation at the very begining to all signers as it's always been my understanding that it would cover any following Jurat. I word the oath as follows...
"Do you swear/affirm that you are the person you claim to be and that the information provided in documents is truthful to best of your knowledge?"
I make sure to leave out any mention of God while circling affirmation on the certificate everytime just to avoid my notarizations being thrown back at me later in court as a result of having a signer swear to God when he's Muslim and believes in Allah or as an Atheist who doesn't believe in anything at all.

Why not just ask "For these documents which you just signed......" one time?
If you just completed 135 notarizations (same person, same time) - the guy is buying lots of deeds (perhaps a mining company) - would you oath him 135 times?

If I'm doing 10 jurats for the same person, I'll do an assembly line Oath procedure (this procedure is not in any notary manuals BTW). One after the next really quick.

Once per person regardless of the number of signatures notarized.
I ask all signers to pay attention to the following question, then point to them individually for their answer.
I ask you all individually:
Do you acknowledge and affirm that these are your signatures, that you understood and willfully signed these documents, and that the statements contained are true and accurate to the best of your knowledge and belief.
Once had a "NO". Lady said she did not have a clue about the documents. Called Title and we agreed to trash the entire package. Title was furious when they found the LO "had not even spoken" to the borrower.

If you have multiple Jurats, do you give an Oath once, or is it once per person per Jurat? And how do you word the Oath? I know the answer, I'm just testing you guys!
