I designed one a little different and more simplistic:

Notary Public
Certified Signing Agent



I, [Name], am before you today as a Notary Public to witness ONLY your signatures on documents. I have in no way given you direction other than where to sign or answered any questions regarding the details of your loan or the documents you are signing. I may steer you to the section you will find your answer on the documents but have in no way answered any detail questions regarding your loan or advised you of what actions to take in filling out the documents before you today.

In addition, I have administered an oath of truthfulness in which you have stated, ‘I do.’
[Do you swear that the statements in these documents are true?]

County of ___________________

Signature of Borrower ________________________________________Date:_______________

Signature of Co-Borrower _____________________________________Date:_______________

They are signing to say they have been advised of my role, I did not advise them and also that I administered the oath.

I'm curious, how many NSAs have actually used a "Hold Harmless
Agreement? How many NSAs WISH in hindsight that he/she had
used one?

I read somewhere (I think in your course) about using a "Hold Harmless Agreement" as protection for the NSA. Do you have a "sample" agreement of this type?
