We just moved down here last May and I got my Florida driver's license in June - they did not require my maiden name - they used the exact name I had on my license in Connecticut, which is first name, middle initial, last name (a good thing because I've been known as that for over thirty years and everything I own is in that name).

I think each county in Florida is different. I know someone who moved down here in 2005 (to Stuart - not sure what county that's in) and he was told it was mandatory that his license contain his middle name fully spelled out - no initials (which he hated!!) - when I mentioned that to the DMV here in Columbia County she'd never heard of such a rule so she checked with others in the office and no one else had ever heard of it.

I've done signings here for people who have their maiden name as their middle name and they've told me the DMV told them they could use their maiden name as their middle name on their license if they wanted to.

As far as I know, they do not require a maiden name. I have had the same Fl drivers license for 18 years, so unless there is a new law, the answer is no

Name Change for United States Citizens

If you have legally changed your name by marriage or court order, you must submit the original or a certified copy of your marriage certificate or court order. No photocopies will be accepted unless certified by the issuing authority.

I wonder if it is true that Florida requires a woman's maiden names on the on the license just curious...
