Florida Notary Acknowledgment / Florida Notary Jurat Information
Here are some sample Florida Acknowledgment and Florida Jurat certificates.  Although notary 
certificates can vary from state to state, if you compare them, you will find many similarities.  
One oddity of this particular Florida Acknowledgment certificate is that the words, "Appeared 
before me" do not appear.  Rather, it is written, "Acknowledged before me".  Please keep 
in mind that certificate forms from any state can change over time, and the information on 
this page could become outdated at any time. 
 
Also note that the Florida Jurat certificate says, "Sworn to (or affirmed)", rather than a more 
typical verbiage, "Subscribed and sworn to before me".  This is a sign of the change many states 
are making to combine the two acts of Oaths and Affirmations into one act.  It used to be a 
different notary act to make an affirmation. The wording was different, as it contained no 
reference to a supreme being (god).  These days, more and more states, including Florida and 
California are combining these acts.
 
Another oddity of the Florida Jurat is to include a statement on the certificate regarding what 
type of identification was used by the signer.  This is a super idea, and perhaps more states 
should follow it.  States that require journals often require the notary to record the ID type 
in the journal, but its not so common to document the identification type on the notary 
certificate itself.
