I tell the borrower that the docs go back to the lender to be destroyed.

I'd never take a signed deed because the final step in making a deed effective is delivery. If the signer gives it to me with the expectation that I will send it to the lender or title, that could be considered delivery.

When the loan doesn't go through, the notary is supposed to grab the documents and run. The borrower can keep their copies. But, have you ever had trouble running off with the documents? Sometimes borrowers are not so agreeable about letting go of what the lender considers to be their property.
