Posted - 04/02/2008 : 4:14:44 PM I need information on how to correctly notarize a medical records affidavit. Who needs to be present? Which form do I use? How much should I charge? Am I just notarizing an oath or signature?

Posted - 07/20/2008 : 5:24:57 PM I have done a bunch of these on a regular basis and they are a dream come true. No wait for the doctor, no wait for your "client". The custodian of records is almost always available. One quick "wack" and it's over. Most of the time the directive is to "leave it there"! Easy as can be, not even a shipping label to complete. Usually the MD's office is on the ground floor, no security checkpoint or waiting for an elevator. I schedule 15 minutes for one of these and usually have time to stop at Starbucks after. Just be sure to call in your completion promptly - and they tend to send the check promptly too!

"Which form do I use?" Not your call - they should provide the forms WITH the notarial wording included on it. If the wording's not on it it's up to them to tell you if they need an acknowledgement or jurat.
"How much should I charge?" $x per signature (I believe in CA you can charge $10/signature notarized) plus whatever you're allowed to charge for services, time & travel.
"Am I just notarizing an oath or signature?" You're always notarizing a signature - you don't notarize an oath - the signer may TAKE an oath or you may administer an oath.
You really need to go back and review your handbook. MHO
