Acknowledgment certificates
Remember, 80% of notarizations are acknowledgments, so carry "loose" acknowledgment certificates. Wording varies from state to state, but they will all have: a venue, the name of the signer, that they appeared before a notary, the name of the notary, and that they signed a document. These forms often use terms like, "executed the instrument"
to say "signed the document". The exact wording is not too important just as long as the key information that I mentioned
is there.
