Permission for a minor to travel accompanied by an adult.
There is no standard form sold for this type of situation. I created my own form and copied it many times and carried it with me
wherever I went. In Los Angeles County, families are sending their kids to Mexico with aunts, uncles and other family members and need
this type of paperwork notarized. Its good to be familiar with this type of situation as its common. Its not a bad idea to be really thorough and have records of the identification of the parent granting permission, as well as of the adult who is going to actually go on the trip. The more documentation, the merrier!
