As I said in the other thread - send all of it - these are legitimate requests. However, if it were me, I would not send a copy of my DL - if they insisted I'd black out any PII it contained, but my policy is I do not provide copies of my DL.

Make sure the W-9 has your Fed. EIN on it - do NOT provide your social security number - get an FEIN at www.irs.gov - takes 5 minutes and you use it for all your business dealings.

Commission so they know you're really a Notary.
W9, tho' EIN is a better choice, so they can pay you.
E&O Insurance if required.
Auto Ins., D. License---nah! They have no need for that from an Independent Contractor. Tho' I might send a copy of DL with
personal info removed if I felt the co. was worth the effort.
